Username: Legal first name and first initial of last name and last 4 of SSN

Alternate Username (if above does not work): Legal last name and first initial of first name and last 4 of SSN

Password: full SSN (no spaces or dashes)

Example Employee: John Doe with SSN of 111-22-3333

Example Username: JohnD3333

Example Alternate Username: DoeJ3333

Example Password: 111223333

*Please note your social security number will only be used for initial login, at which time you will be prompted to change it.
